AmeraCare Home Health is proud to introduce Safe Steps, an initiative aimed at promoting fall prevention awareness and mobility rehabilitation.

The Centers for Disease Control and Prevention report:
  • Over 95% of hip fractures are caused by falls. a
  • One out of three adults, aged 65 or older, falls each year but less than half talk to their healthcare providers about it. a
  • Falls are the leading cause of both fatal and nonfatal injuries in older adults. a
  • The death rates from falls among older men and women have risen sharply over the past decade. a
Safe Steps employs a multi-faceted approach to treatment:


    • AmeraCare’s interdisciplinary team members receive specialized, on-going fall prevention and rehabilitation training.
    • Patient and caregiver educational materials are provided, reviewed, and discussed during the initial skilled nurse visit.
    Coordination of Care:
    • Each patient is assigned a proactive case manager to lead and coordinate the interdisciplinary team
    • A mobile, electronic documentation system allows clinicians to reference the patient’s medical record during patient visits
    • The case manager and medical director meet biweekly to evaluate each patient’s plan of care.
    Screening, Assessment & Intervention:
    • From admission to discharge, a holistic approach to treatment is utilized to address the physical, practical, emotional, and social dimensions of the issue.
    • All patients are screened for fall risk.
    • Both quantitative and qualitative screening and assessment instruments are administered.
    • Those “at risk” for falls are evaluated by the physical therapist and a physical therapy plan of care is established.
    • An effective assessment of orthostatic hypotension is provided.
    • Targeted, coordinated, and interdisciplinary responses to each fall are case manager driven.
